In an effort to save the patient time at the initial appointment, the
Patient Intake Form and System Survey are included so that the patient may complete the forms prior to the first
appointment. If the patient is taking medications, a list of those medications should be included in the Intake Form.
Completed forms may be emailed or faxed to the Clinic prior to the appointment, or may be brought in at the time of the appointment.
